Agreement and Consent Forms for Services Provided

Below, you will find the necessary agreements for accessing our services along with consent forms tailored specifically for virtual counseling and advocacy services. These documents are essential to ensuring that you receive the support you need safely and confidentially.

  • DARCC offers comprehensive services for those whose lives have been affected by sexual violence.

    Services are available to all survivors 13 years and older, as well as to individuals impacted by sexual violence through their relationship with a survivor.

  • Virtual simply means that services are administered through internet-based technology. Appointments will be held through your smartphone or computer using Zoom, a HIPAA-compliant service.

Civil Rights Statement

Dallas Area Rape Crisis Center assists sexual violence survivors—regardless of sex, gender identity, race, physical/developmental disabilities, income, ethnicity, class, religion, sexual orientation, and former/current engagement with sex work.

We offer information in both English and Spanish, as well as interpretation services for most other languages upon request.

Confidentiality

Dallas Area Rape Crisis Center takes our privacy policy seriously. Our organization maintains confidential information to ensure the safety of victims; victims’ families; volunteers; employees; donors; and others related to the program. We do not sell or otherwise disclose information about our clients, volunteers, or supporters outside of our immediate organization. This policy has no exceptions. 

Privacy Policy

Dallas Area Rape Crisis Center (“us,” “we,” or “our,”) operates darcc.org (the “Site”). This page informs you of our policies regarding the collection, use, and disclosure of Personal Information we receive from Site users. We use your Personal Information only to improve the Site. By using the Site, you agree to the collection and use of information by this policy. 

Information Collection and Use  

While using our Site, we may ask you to provide us with certain personally identifiable information that can be used to contact or identify you.
